Definition A chronic functional disorder that manifests by alteration in bowel habits, and recurrent abdominal pain and distention.
   
Prognosis Recurrences are the norm throughout life, especially at times of stress, though with less frequency with advancing age. Does not progress to cancer or inflammatory bowel disease some improve over initial one year and remain well.
   
Treatment Irritable bowel syndrome

High roughage diet with fruits and vegetables.
Tab. COLOSPA 100mg tds (Mebeverine) or Tab. Spasmapriv 100 mg tds, If Dysentry is the prominent symptom.
Tab. Eldicet 50 mg 1 bd to tds If abdominal pain cramps is  the prominent symptom.
Tab. Imosec 2mg 1 bd to tds (Loperamide) or Tab. Lomotil 2-5 mg. bd.
If stools are small and hard, Isaphgul 2 tsp at bed time.
Tab. DEPSONIL 25g. tds x 30 or Cap. Fluoxetine 20mg.

General Measures Avoiding dietary triggers, reducing stress, increasing fibre (25 gms. Day) in the diet in the constipation-prone case, avoiding large meals especially spicy or deep fried.
Heat to abdomen will help.
Biofeedback has been tried with some success.
   
Advice to Patient Avoid triggers.
Be reassured on absence of any organic or known bio chemical basis for the illness and the condition never progresses to caner or inflammatory disease.
Expect recurrences throughout life when under stress, but the problems gets less frequent with advancing age.
   
Follow Up As needed for symptoms.
   
Inadequate Response Reassess if convinced of any new organic problem arising.
Psychiatric counseling and therapy, if needed.
   
Prevention Of triggers if known
